The facility agrees to provide the following services which are included with your monthly payment...

	A private room which includes a bed, bedside table, and lamp, chair, dresser, bath linens, bed linens and blinds.
	Meals which include three meals a day and additional snacks.
	Personal care services which include assistance with eating, personal hygiene, transferring, toileting and dressing.
	Laundry and housekeeping services.
	Assistance with access to healthcare, social services, and social activities.
	Reminders or physical assistance to residents who can self administer medications (and/or administration of medications.)
